Bridge or implant?

A bridge is quicker and does not need surgery, but involves reshaping healthy teeth. An implant leaves the neighbours alone and often lasts longer, but takes months and costs more up front. Which is better depends on the state of the teeth either side. If they already have large fillings and would benefit from crowning anyway, a bridge makes more sense than it otherwise would.

How a bridge works

A bridge replaces a missing tooth by anchoring a false tooth to the teeth either side of the gap. The supporting teeth are called abutments and the replacement tooth is called a pontic. The whole thing is cemented in place, so unlike a denture it does not come out.

Types of bridge

  • Conventional bridge. Crowns on the teeth either side with the replacement tooth joined between them. The most common type, and it means reshaping both supporting teeth.
  • Cantilever bridge. Supported on one side only, used where there is a suitable tooth on just one side of the gap.
  • Resin-bonded bridge (Maryland bridge). The replacement tooth is held by thin metal or ceramic wings bonded to the back of the adjacent teeth. Much less tooth is removed, which makes it attractive for front teeth, though it is more likely to come loose over time.
  • Implant-supported bridge. Held by implants rather than natural teeth, so the neighbouring teeth are untouched. Useful where several teeth in a row are missing.

How a bridge is fitted

Two appointments in most cases. The supporting teeth are numbed and shaped, an impression or scan is taken, the shade is matched and a temporary bridge is fitted. At the second visit the permanent bridge is tried in, checked for fit and bite, and cemented.

A new bridge feels bulky for a few days, particularly the underside of the pontic where it meets the gum. That sensation fades as you get used to it.

Cleaning under a bridge

This is the part that determines how long a bridge lasts, and it is different from cleaning natural teeth. Floss cannot be passed down between the units, because they are joined, so you clean underneath instead, using a floss threader, superfloss, or a small interdental brush passed beneath the pontic.

Plaque left under the pontic causes gum inflammation, and decay at the margins of the supporting crowns is the most common reason bridges fail. We will show you the technique when the bridge is fitted, and check it at each visit.

What a dental bridge costs in Ontario

Ontario dentists set their own fees, and most use the Ontario Dental Association's annual Suggested Fee Guide as a reference point. The fee for any given treatment depends on how much work is involved, the materials used, and whether anything else needs doing first, so a written estimate after an examination is the only figure worth relying on.

A bridge is usually quoted per unit — each supporting crown and the replacement tooth counts as one. We will give you a written treatment plan with the fee codes before anything goes ahead, so you can send it to your insurer for a pre-determination and know what you will be paying before you commit.
